Our chiropractors are trained to actively communicate with other healthcare providers, including medical doctors, about patient needs. Our Chiropractors area well trained to refer patients who are presenting with non-musculoskeletal complaints and understand when there are contraindications to manual therapy.
Making a patient referral is simple, easy and quick and it ensures that your patient is receiving the best possible care. Chiropractors receiving referrals typically respond with a referral/reply note and/or progress note.

All our chiropractors support and accept referrals for the Medicare funded Chronic Disease Management program. Registered with the Department of Human services and the Chiropractic Board of Australia our Chiropractors meet the Medicare Professional Eligibility to provide Chiropractic Allied Health Services for Chronic Disease Management.
For the initial consultation, there may be a gap fee for this service.
For subsequent consultation, we accept the Medicare benefit as full payment for the service, there will be no out-of-pocket cost.
For GP’s wishing to make a Medicare referral please complete the following referral form found at the Department of Health website.
Referral to nominated Chiropractor
GP’s can choose to refer to our Medicare-eligible Chiropractor registered with the Department of Human Services. When completing the referral form for Allied Health Services, please enter the Practitioners Name, Address and number of services required.
Alternatively, to arrange the earliest available appointment for your patient we recommend an “open referral”. With an open referral you do not need to nominate a specific practitioner, but will reference the Practice name (MyChiro Floreat) in the Allied Health Provider Name section of the referral form. This will allow your patient maximum flexibility to consult with a Medicare eligible chiropractor at the earliest availability and most convenient time.
On completion please forward the Referral form, GP Management Plan (item 721) and Team Care Arrangements (item 723) to the contact details below.
Medicare referral arrangements are required to be received by our office a minimum of 24 hours prior to the patients initial consultation.

We proudly accept veterans for chiropractic treatments. Veterans are covered by Department of Veterans Affairs (DVA) for chiropractic services.
A Referral form and DVA health card is required at the time of consultation. The cost is then billed directly to DVA.
